All requests for appointments will be reviewed by a GP before an appointment is given. Please complete an electronic consultation form below. Your form will be reviewed by a GP and we will be in touch with the outcome of the review which may include a face to face appointment at the practice.

- visit the surgery and speak with a receptionist during our opening hours
- go online and put through an online consultation

Cancelling or changing an appointment

- use our online system at any time (find out how to get access)
- contact us during our opening hours
If you need help with your appointment
Please tell us:
- if there’s a specific doctor, nurse or other health professional you would prefer to respond, this may mean a longer wait for the review but if it is an ongoing problem continuity can be really helpful
- if you need an interpreter
- if you have any other access or communication needs
Home visits
If you are housebound and you have a medical need, we’ll do a home visit. We’ll phone you first to understand what you need. Your home visit will likely be a member of our home visiting team which includes nurse practitioners and paramedic practitioners.
To request a home visit, it’s helpful if you phone the surgery as soon after we open possible, as home visits must be requested before 11.30am.
