WELCOME

Our list is open. The information on this page will enable you to understand how to register as a patient at this practice and give information on the rights and responsibilities of patients.      

When registering at the practice, please bring your NHS Medical Certificate to the practice and those of any other persons registering with you.

If you do not have a medical card, you will be asked to complete a registration form for each person registering with the practice.

In addition, all patients will be asked to complete a new patient
questionnaire, allowing us to provide medical care in the interim period while your medical records are transferred from your old practice to this one.  You will also be asked to complete and sign a Patient Contract.

If you move out of the practice area, you will need to register with a new practice as soon as you move. We can provide information about how to find a new practice.

In order to register at our surgery you will need to complete the following documents:

  • New patient Questionnaire
  • Patient Contract
  • GMS1 Form
  • Application Form for online services

Please bring the completed documents with some form of identification - preferably a utility bill, driving licence or passport to the surgery and our admin team will start the registration process for you.

Each patient has a named accountable GP for the purposes of clinical oversight, but you may request an appointment with any of our doctors.

Temporary Residents

If you are staying in Sutton upon Hull area, we are happy to see you in our normal surgery appointments, either with doctors or nurses.

You will be asked to complete a simple form, recording your own doctor as well as your permanent or temporary address. If you are staying more than 3 months, you should register as a permanent patient.

Visitors from abroad are welcome. European and Commonwealth residents can expect full access to free NHS emergency care. Some countries do not have such arrangements with the NHS, and so patients can expect to be charged for consultations. 

As a temporary patient, we will not have access to your medical records so will be restricted in what log-term care we can offer. This will be discussed with you at your appointment but is usually best left to your own doctor.

Overview

We are required under our contract to allocate a named accountable GP for all patients.

However, you may still see whichever GP you prefer in the usual way.  If you are not already aware of your named accountable GP, this information is available on repeat medication slips or by simply by requesting the information from the surgery.

Accessible Information Standard

The Accessible information standard (AIS) is an NHS England information standard which must be implemented by all organisations that provide NHS or adult social care.

The AIS aims to ensure that people who have a disability, impairment or sensory loss receive information that they can access and understand - for example in large print, braile, professional communications support if they need it , such as a British Sign Language interpreter.

Change of Address/Details

No changes to Dates of Birth can be accepted online - proof must always be provided for these (e.g. a passport or birth certificate). Please attend the Practice to complete a form for this.

Changes of name - other than for reasons of marriage - must also be submitted using a paper form at the Surgery, with proof provided.

·Please remember to inform any other Hospitals, Clinics and other healthcare providers of your change of details if you are currently under treatment with them.

The online Change of Address Form below  may be used to change the address or contact details for yourself and other members of your family. Please note that it may take several working days for your details to be updated on our Clinical System
